The Power of Choice
Where a person lives is central to who they are, and we strive to provide the people we serve with choices–from the community they wish to build a life in to the house they want to call home. That’s why, in addition to our Lifesharing program, we also offer group home settings in our Community Living program.
Lifesharing Program
In our Lifesharing program, also known as Host Homes, adults with intellectual and developmental disabilities live in the community of their choice in the private family home of their dedicated Lifesharing providers who we call Mentors. We call them Mentors because they are more than caregivers–they are teachers, advocates, and friends.
Personalized Supports

They can learn new skills, discover new hobbies, and explore their community–all while receiving the supports they need. With round-the-clock guidance from our expert team, Mentors provide the individuals we serve with:
- 24-hour home-based monitoring
- Medication management oversight

- Transportation
- 24/7 on-call support from our team of experts
Our Lifesharing program’s innovative approach gives the adults we serve an invaluable sense of home, community and family, giving them the chance to live life to the fullest every day.
It’s All about Relationships
We carefully select our Mentors and ensure they have the skills needed to provide exemplary support to the people we serve. Once a Mentor is ready to welcome an individual into their home, we seek the best match possible by considering the needs and preferences of both the Mentor and the individual, helping to ensure the placement evolves into a long and enriching relationship.
A Community Solution
The Lifesharing model’s innovative design is extremely flexible and offers highly personalized support. For the individuals and their families, our Lifesharing program provides invaluable peace of mind. For the payors supporting them, it represents a reliable, affordable, and quality-focused solution that eliminates the need for capital expenses and the worry of ensuring continuity of direct care staff.
